I, the undersigned, acknowledge that participation in the Destination Wellness of Licking County 5k Walk/Run in Support of SASS for Breast Cancer in Licking County is a potentially hazardous activity. I attest that I am physically fit and sufficiently trained to participate in this event. I understand that participation may involve risks, including but not limited to:
•Falls
•Contact with other participants
•Effects of weather
•Conditions of the course
In consideration of your acceptance of my entry, I, for myself and my heirs, executors, administrators, and assigns, do hereby release, discharge, and hold harmless Destination Wellness, its affiliates, sponsors, volunteers, and all other participants from any and all claims, damages, or liabilities arising from my participation in this event.
I CONSENT to receive medical treatment that may be deemed advisable as a result of any injuries I sustain before, during, and after this race and agree that I am solely responsible for all costs, including diagnosis, treatment, medical transportation, and evacuation arising from this race.
I grant permission for the use of my name and photograph in any promotional materials related to this event.
I CERTIFY that I have read this document, understand its contents in their entirety and have executed this document below without reservation or duress.
This Waiver and Release from liability contains the entire agreement among the parties relative to the subject matter herein and supersedes all prior or contemporaneous promises, agreements, representations, and understandings, whether written or oral, of the parties with respect to the subject matter hereof.
